Annual Wellness Visits

Your annual wellness appointment is an important part of the care we provide here at Family Physicians of Evans. 

We have a phenomenal team of wellness coordinators that will conduct your visit and work with you to go beyond treatment and delve into preventive health care. With a focus on prevention and not intervention, you get a unique opportunity to address any risk factors that may be impacting your health and quality of life overall. 

During your visit, our wellness coordinator will review your history, discuss your specialists, review your medications, perform a health risk assessment, provide personalized health advice, and develop a schedule and plan for appropriate preventive services.
